in Wautoma, Heritage House in Portage, Patriot Place in Berlin,
Prairie Place in Ripon and Preston Place in Redgranite.
Each facility is licensed by the State
of Wisconsin Department of Health & Family Services as a Community
Based Residential Facility (CBRF) and/or a Residential Care
Apartment Complex (RCAC).
Room accomodations are as follows:
Heartland House: 4 studio apartments (480 sq. ft.); 4 two bedroom apartments (842 sq. ft.); 33 one bedroom apartments (575 sq. ft.); and 20 CBRF private rooms (240 sq. ft.).
Heritage House: 4 one bedroom apartments (720 sq. ft.); 26 one bedroom apartments (676 sq. ft.); and 36 one bedroom apartments (575 sq. ft.).
Patriot Place: 2 two bedroom apartments (842 sq. ft.); 51 one bedroom apartments (575 - 768 sq. ft.); and 20 CBRF private rooms (375 sq. ft.).
Prairie Place: 42 one bedroom apartments (575 - 768 sq. ft.); and 20 CBRF private rooms (375 sq. ft.).
Preston Place: 4 studio apartments (480 sq. ft.); 4 two bedroom apartments (842 sq. ft.); 32 one bedroom apartments (575 sq. ft.); and 20 CBRF private rooms (375 sq. ft.).
Each facility is handicap
accessible throughout with
modern fire and safety features. There is mail delivery and pickup in the facility, spacious central dining area, lounge areas, centrally located laundry facilities, beauty/barber shop, emergency call system for accessing staff and individually controlled heat in all rooms. At least 26 satellite television channels are included. Security deposits are not required.
The care and well-being of
each resident is our primary goal, and to accomplish this goal we offer a variety of services personalized to meet the needs of each resident.
All services are provided by trained staff who respect the dignity and privacy of each resident.
trained staff 24 hours 7 days /week, individualized case management, medication administration and monitoring, diabetic and colostomy care, personalized assistance with bathing, dressing, grooming, treatments, transfers, ambulation, help with transportation arrangements, multi-denominational church services, pharmacy delivery services, regularly scheduled activities, three meals daily, and laundry and housekeeping services.
